This article walks through how to create a credentialing form for volunteer coaches, team managers, or other volunteer staff roles. This form can collect waivers, required documents, and onboarding information.
Before You Start
Make sure:
You know whether the credentialing form should apply to all programs or only specific programs.
You understand which documents or waivers volunteers must complete.
Create a Credentialing Template for Volunteers
In the left navigation menu, go to Staff > Credentialing.
Click + Add Credentialing Template in the top right corner.
Enter a clear Title, such as “Volunteer Coach” or “Volunteer Team Manager.”
Check the box labeled For Volunteering.
From the Role(s) dropdown, select the volunteer role or roles that should complete this form.
If the form should only apply to certain programs, select them in the Program(s) dropdown.
Click Save.
Once saved, add:
Required information fields
Waivers
Document upload requirements
Any necessary eligibility window or submission dates
The role you select here must match the role you assign in your Volunteer Option. This mapping tells the system which credentialing form to display after registration.
If You Run Into Issues
If the credentialing form is not appearing for parents:
Confirm the template is marked For Volunteering.
Confirm the selected role matches the role used in your Volunteer Option.
Confirm the credentialing form dates are active.
