Configuring Player Evaluation Forms

Learn how to create and assign Tryout Evaluation forms so evaluators can rate specific skills during tryouts.

If you want evaluators to rate specific skills (instead of only using an overall rating and ranking), you can assign a Tryout Evaluation form to your tryout registrations.

Before you begin

  • Player forms are created from Players > Forms.

  • When creating a tryout form, select Tryout Evaluation (not Season Goals & Progress).

  • You can assign multiple evaluation forms to the same tryout registration.

Step 1: Create a Tryout Evaluation form

  1. In the left navigation, go to Players.

  2. Select Forms.

  3. Select Add player form or Duplicate an existing form.

  4. When prompted for the form type, select Tryout Evaluation.

  5. Build your criteria and form layout.

Step 2: Assign the form to tryout registrations

After the form is created, you will assign it to the registration or registrations you want evaluators to use.

  1. Open the evaluation form.

  2. Select the Assignments tab.

  3. Click + Add Assignment(s).

  4. Select the appropriate registration from your Tryouts program.

  5. Set the Start Date and End Date (optional, but recommended).

  6. Save.

Update an assignment later

If you need to change which registration the form is assigned to, you can return to the Assignments tab at any time and update it.

Step 3: Control when evaluators can use the form

Assignments include Start Date and End Date columns. These dates control when the form is available for evaluators to view and use.

This is helpful if you want to:

  • Prevent early scoring before tryouts begin

  • Close evaluations after a set deadline

If you run into issues

  • If evaluators cannot see the form, confirm it is assigned to the correct registration and that today falls within the Start Date and End Date range.

  • If the wrong form type was selected, create a new form and make sure you choose Tryout Evaluation instead of Season Goals & Progress.

  • If you updated assignments but nothing changed for evaluators, double check you edited the correct form and the correct registration row under Assignments.

Where do I create a Tryout Evaluation form?
