Learn how to generate bulk waiver PDFs using Sprocket’s PDF Form tool, including how to upload and map fields and download completed waivers for recordkeeping.

Generate a Waiver PDF Form

If your club needs a downloadable copy of completed waivers (for example, for audits, travel paperwork, or offline recordkeeping), you can generate waivers in bulk using Sprocket’s PDF Form tool.

This is a useful best practice for Programs and Registrations, but the setup and generation steps live under Waivers in the left navigation.

Before You Start

Before waivers can be generated out of the system, the PDF version of the waiver must be created with labeled fields in each area that should be populated from Sprocket.

If you need assistance creating the PDF with the correct labeled fields, reach out to your assigned Client Success Team.

Upload a PDF and Map Fields

  1. Navigate to Waivers in the left navigation menu.

  2. Click into the waiver you need to generate.

  3. Toggle to Form on the top menu bar.

  4. Click Upload PDF Form and upload your PDF.

  5. Based on the fields on the PDF, the corresponding field labels will appear in Sprocket.

  6. Use the dropdown next to each field label to connect the corresponding Sprocket field.

Please note: Sprocket fields can only be tied to one field label.

  1. Once all fields are tied, click Save.

Generate Waivers for Players

  1. When you are ready to generate, click Generate Form. This enables the player selection module.

  2. Filter to the necessary players and click Generate.

  3. A batch PDF of the waivers will download once complete.

Tip

If you plan to generate waivers regularly, keep your PDF field labels consistent and your Sprocket field mappings clean. This reduces setup work when updating waiver versions season over season.

Frequently Asked Questions

Can I generate waivers without uploading a PDF form first?

No. The PDF version of the waiver must be uploaded and mapped with labeled fields before waivers can be generated.

Why are my waiver fields not populating in the PDF?

Each field in the PDF must be properly labeled and connected to a corresponding Sprocket field. Also note that Sprocket fields can only be tied to one field label.

Can I generate waivers for multiple players at once?

Yes. After clicking Generate Form, you can filter and select multiple players, and a batch PDF will download once complete.

Who should I contact if I need help setting up the PDF form?

Reach out to your assigned Client Success Team for assistance creating or mapping the PDF with the correct labeled fields.
